Major duties

• The LPN is responsible for assisting the PACT with ongoing, continuous care of one of more assigned panel of Veterans. This LPN is responsible for basic LPN duties as listed below and other duties as assigned: Provide direct patient care. Monitoring vital signs. Observes, records and reports patient's response or changes in patient's condition. Assisting the provider with exam/procedures. Keeping Veterans advised of delay/wait times. Reviewing daily clinic schedule for patient care needs. Promptly recognizes and responds to urgent or emergent situations. Checks daily equipment/supplies and restocks the exam rooms. Collaborate with team to develop comprehensive health care plans and care management plans for patients. Acts as patient advocate. Assures patient understands content of visit and when to return to clinic. Requirements

You must be a U.S. Citizen to apply for this job. Education

Preferred Experience: 1 year preferred experience in a healthcare setting and experience with video conferencing or other telehealth technology. Patient Aligned Care Team (PACT) model of care experience, cerner millennium electronic health record experience and phlebotomy experience. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-3 to GS-6.
